These are big questions with no authentic answer, however, I will try to answer these questions with some accuracy to the best of my knowledge of treating Kidney Stones for around 40 years now and to help a Kidney Stone patient understand Kidney Stones better – Dr Pawan K. Gupta

Unnoticed Kidney Stones cannot be treated as patient as well as the doctor will never know about them.
All Kidney Stones are not the same,
so is the treatment

The Treatment of Kidney Stones will depend on various factors like: –

  • Size of the Kidney Stone.
  • Location of the Kidney Stone.
  • Type of the Kidney Stone
  • Infected and non-infected Kidney Stones
  • Kidney Stones in anomalous kidneys such as horseshoe kidney, crossed ectopic kidney, simple ectopic kidney, pelvic ectopic kidney, duplex system and malrotated kidney
  • Kidney Stones with associated comorbidities. Comorbidities mean the other diseases present in a patient which can influence the treatment of kidney stone. These comorbidities include obesity, Bleeding disorders, chronic renal failure, Hypertension, Diabetes and several other diseases.
  • Treatment of the Kidney Stones can also depend upon the extent of facilities available at a particular location.
  • The requirement of emergency Intervention which may include placement of Double J Stent or Nephrostomy Tube to decompress the Kidney.

  • Kidney stone treatment has seen dramatic changes in the last three decades. Open Kidney Stone Surgery was replaced by Extracorporeal Shock Wave Lithotripsy & Percutaneous Surgery of Kidney Stones in Nineteen eighties & the incidence of Open Kidney Stone Surgery was reduced to 1 to 2 %. Similarly, Open Surgery for Ureteric Stones was replaced by rigid Ureteroscopy.
  • Extracorporeal Shock Wave Lithotripsy
    Kidney stone treatment by SWL Has a limited but definite use today. It should be used for stones of around one cm. preferably in the upper part of the Kidney.
  • Percutaneous Surgery of Kidney Stones
    This is a widely practised Keyhole surgery for Kidney stones & has good results particularly for large stones in the Kidney. Surgery has a significant complication rate.
  • Flexible Ureteroscopy / Retrograde Intrarenal Surgery is the best treatment modality for treating Kidney Stones
    During the last decade, a new modality of treatment in the form of Flexible Ureteroscopy / Retrograde intrarenal Surgery is picking up fast for treatment of Kidney Stones. This procedure is a natural hole surgery & is associated with minimal complications. The procedure is expensive because of the higher cost of Equipment & disposables & also has a long learning curve.

In the present time, “RIRS” is the best procedure for the removal of kidney stones. RIRS is an abbreviation for Retrograde Intra Renal Surgery. This is a Natural hole, No cut, No artificial hole, Daycare & Safest procedure for kidney stone removal.
